Use this form to let us know if you are a veteran. It is important that we know if you are a veteran so that we can make sure you receive the appropriate information, services, and support available.
A veteran is anyone who has served for at least one day in HM Armed Forces, whether as a Regular or Reservist (previously known as the Territorial Army). It means the same as ‘ex service personnel’ or ‘ex forces’.
You can use this service if you:
- are registered at the surgery
- have ever served in HM Forces, either Regular or Reservist (previously known as the Territorial Army)
