New patient registration
To register with the surgery:
- use the NHS Register with a GP Surgery service
If you prefer to complete a paper version of the registration form one can be collected from the surgery or downloaded from the NHS Register with a GP Surgery service
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We aim to process all registrations as soon as possible but it can take a few weeks for all your information to be transferred to us from your previous practice. If you are on regular medication, please make sure you have had a recent prescription from your current GP before you start the new registration process. We will be able to offer urgent care from the time you submit the forms but may not have sight of your medication history until the electronic transfer of your records has been completed.
Registering with Lydney Surgery
Once you are registered with the practice, we highly recommend you register for NHS services.
Download the NHS App, or open the NHS website in a web browser, to set up and log in to your NHS account. Owned and run by the NHS, your NHS account is a simple and secure way to access a range of NHS services online. On the NHS Services you will be able to order repeat medication and get health information and advice.
It is a requirement that any changes in personal details such as name, address, contact telephone numbers etc. are communicated to the practice promptly.
You can let us know via our change personal details form. Please note that if you move out of the practice area you will be required to register at an alternative surgery.
